Congestive Heart Failure/COPD

Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD) are chronic conditions affecting millions. CHF is the underlying reason for 6.5 million hospital admissions each year. Research has shown that education can reduce the frequency of hospital admissions by teaching individuals and loved ones how to cope with the day-to-day challenges of CHF and COPD.

Working with our physicians, we have designed a program with the goal of reducing hospitalizations and of maintaining physical conditioning. This is an education and therapy —based program designed to help:

  • Reduce hospitalizations
  • Strengthen exercise capacity and endurance
  • Improve abilities to carry out daily living tasks
  • Understand breathing techniques and control
  • Increase energy levels and stamina
  • Monitor and control hypertension
  • Learn medication management
  • Maintain balanced nutrition
